Enrollment & Rules To Attend To ensure the safety and well being of all pets at Downtown Dogz the following is required for enrollment: We accept dogs of all ages puppies must be at least 16 weeks of age. If your dog is over 7 months of age, he or she must be neutered or spayed. All dogs must be current on DHLPP, Rabies and Bordatella vaccinations. Each dog must undergo an initial behavior assessment to determine enrollment eligibility. Downtown Dogz reserves the right to turn away any dog unsuitable for this environment. Signed waivers are required for enrollment. Application Materials You can download our application materials directly from our Web site. Save time by printing and filling out these documents before you come visit us. Rules to Attend Daycare · All paperwork must be completed (emailed, mailed, dropped off) on or before an evaluation day can be scheduled. · The evaluation must be completed before your dog can start attending day care or spending the night. · Dogs must be at least 16 weeks of age · Must have no history of aggressive behavior towards people, other dogs or toys. · No communicable illness with in the past 30 days. · Must be free of ticks and fleas and be on a flea/tick program. · Any dog can be denied admittance during the evaluation process and any dog can be expelled from day care for any behavior deemed inappropriate by Downtown Dogz management. · Common reasons for dogs to fail: -Aggressive Behavior -Excessive mounting -Biting other dogs -Biting staff -Severe medical problems -Too nervous or fearful -If we feel it's not a good fit Enrollment Form Release Form |
